CONSENT & RELEASE AGREEMENT

I, the undersigned Owner (or Guardian) of my pet, hereby grant my consent for the holistic pet consultation and services provided by Dr. Love.

It is important to note that Dr. Love does not provide diagnoses or prescribe Western pharmaceutical medications. The coaching and services offered are complementary in nature and should not be considered a replacement for your routine veterinary care. They do not establish a veterinary client-patient relationship.

Dr. Love's primary objective is to enhance the overall well-being of your pet and manage their health from a holistic perspective whenever feasible. However, it is essential to acknowledge that no specific guarantees have been provided regarding the outcomes that may be achieved, and I am fully responsible for ensuring that my pet receives regular veterinary examinations.

Cost and Payment: I understand that fees may apply for the consultation and services provided by Dr. Love. Payment for these services will be due prior to the scheduled appointment.

 

Emergency Procedures: In the event of a pet emergency, I acknowledge that Dr. Love is not an emergency service provider. In such cases, I will promptly contact a licensed veterinarian or emergency clinic for immediate assistance.

 

Liability and Release: I release Dr. Love from any liability in connection with the holistic pet consultation and services provided to my pet. I understand that holistic treatments can have varying effects on pets, and any unforeseen outcomes are not the responsibility of Dr. Love.

By signing this statement, I affirm that I have read, understood, and agreed to the terms and conditions outlined herein.
